Identification of Unmet Needs for Environmental Supports in Housing and Communities of Georgia’s Seniors
If you are 60 years old or older, we invite you to participate in a research study being conducted by the Center for Assistive Technology and Environmental Access (CATEA) at Georgia Tech for the Georgia Council on Aging. The study involves completing a short survey about the kinds of barriers and supports that you experience at home and in your community while completing your daily activities.

The purpose of this survey is to inform policy makers in the State of Georgia about your needs for supportive design features, products, technologies and social support services in your home and community. This information will be used to help policymakers better determine how to allocate money for senior services.

Your participation is strictly voluntary and you are not required to participate. Please feel free to save a copy of this page for your records. You do not waive any of your legal rights by completing this survey.

The risks involved in completing this survey are no greater than those in daily activities. You will not benefit directly from participating in this survey. Indirect benefits may include the opportunity to participate in research and to improve the products and services used by you and others. You will receive no payment for completing the survey and there are no costs for participating.

All information that is collected is confidential and will only be accessible to research staff. Your name and any other personal identifying information will not be collected from this web survey. If we happen to know your name or other personal identifying information during the recruitment, they will not be directly linked to your responses or appear in any publication, either. Your privacy will be protected to the extent allowed by law.

You should be aware that the survey is not being run from a ‘secure’ https server of the kind typically used to handle credit card transactions, so there is a small possibility that responses could be viewed by unauthorized third parties such as computer hackers. In general, the web page software will log as header lines the IP address of the machine you use to access this page, e.g.,102.403.506.807, but otherwise no other information will be stored unless you explicitly enter it.
