This survey is about chronic illness in the workplace. The questions in this survey will ask you about work, your supervisor, and your well-being in general, as well as about working with chronic illness, including stigma and disclosure issues. This page is meant to provide you with important information about your participation.
• This survey is completely anonymous. You will not be asked to provide any identifying information. There will be no way to link your responses back to you, so you should feel free to respond candidly. (However, if your workplace monitors your internet usage, you may choose to take this survey from your home computer.)
• The entire survey will take around 20-25 minutes of your time. To compensate you for your time, you will be offered the chance to enter a drawing to win one of ten $50 online gift certificates to Amazon.com. To enter the drawing, you will be brought to a separate web page where you may enter your email address after submitting your survey responses. Your email address will never be connected to your survey responses and your email address will never be shared with anyone.
• Your participation will help highlight issues that workers with chronic illness face. Very little research examines issues for workers with chronic illness. I hope to publish the findings of this study in order to advance people’s awareness of these issues.
• Participation in this study is voluntary. You may choose to skip any questions you do not want to answer. You also may also terminate your participation at any time without consequences or penalties of any kind. If you decide during the study that you no longer want to participate, simply close your Internet browser and your responses will not be recorded.
• This study was approved by the University of Connecticut Institutional Review Board (IRB), a group of professionals who monitor studies to protect the rights of participants.
